The Healthcare Teams Pilot Program will address health needs by providing K-12 student-centered care in the school setting using the healthcare team approach to collaborate and coordinate care.
The Utah State Board of Education (USBE) will select two LEAs to participate. Awarded funds will be used as a compensation for up to two team leads per LEA, ($7,000 per team lead, up to $14,000 total), to oversee the implementation of a healthcare team in the school(s). For example: a charter school may apply for this grant, but will only be eligible for $7,000 for the team lead. Each team lead may only have one pilot school. So, if an LEA chooses to pilot in two schools, they must have two team leads, one for each school.
