Requesting Records

The USOE accepts personal and third party GED records requests for GED® Tests taken in the state of Utah. Utah GED Test scores are the property of the Utah State Office of Education. Verification, transcripts or scores will not be released to anyone without written permission from the tester.

To obtain verification, transcripts or scores for tests taken outside of Utah, contact the state in which the tests were taken. The Out-of-State Request Form may be printed and sent to the state holding the record. If you tested previously in the state of Utah and are completing the GED® tests in another state, submit the Records Request Form. Provide the name, address, and fax number of the state where the records are to be sent.

For all other requests, submit in writing via mail or fax to:

Requests will be processed within 7-10 business days from receipt of request. Requests must contain all required information. Incomplete requests will be returned to requester. In addition to the required elements, please list the mailing address or fax number where the GED verification, transcript or scores are to be returned.

Requesting GED Record Changes

Name Changes

The Utah State Office of Education recognizes the right of the individual to change his or her name. However, the Utah State Office of Education reserves the right to refuse to honor a request for name changes on GED records if there are reasonable grounds to believe the request is intended to promote any fraudulent or unlawful purpose.

The Utah State Office of Education assumes no responsibility and is not liable for any adverse consequences to any individual, including any negative outcomes caused by mistakes in recording or providing information that may occur as a result of an individual’s action to change that person’s name as it appears on GED records.

  1. Name changes are allowed due to marriage only when the test-taker is in the process of testing and both their birth certificate and their marriage license are provided.
  2. Name changes are not permitted after the test-taker completes the GED Testing process unless there is a court order that shows a change in name. If this is the case, the candidate must provide the following information to the State GED Tests Administrator:
  • Copy of the official court order, and
  • A government issued form of identification.

Date of Birth Changes

  1. Date of birth changes are allowed if the record holder has provided sufficient information to prove the record in question belongs to them.
  2. The individual must provide the USOE with a copy of a legal document showing the official date of birth (i.e., birth certificate, other government issued ID).
     

Social Security Number Changes

The Utah State Office of Education assumes no responsibility and is not liable for any adverse consequences to any individual, including any negative outcomes caused by mistakes in recording or providing information that may occur as a result of an individual’s action to change that person’s information as it appears on GED records.

  1. Social Security Number changes are allowed if the record holder has provided sufficient information to prove the record in question belongs to them.
  2. The individual must provide the USOE with a copy of their Social Security card and a copy of a government issued photo ID with
