Utah Educators Data Requests
The Utah State Office of Education may provide public educator data to third party requestors. Any individual that would like to request an extract of data from CACTUS should review the data request policy below and then submit the USOE Teacher Data Request Form to the Educator Quality and Licensing Data Steward. Emailing the form is preferred, but mailed or faxed requests are acceptable.
